Shriners Children's Ohio has an opportunity for a part-time Social Worker for our Care Management.  Join a team of professionals committed to improving children's lives.  Our top healthcare professionals excel in their practices and provide compassionate, family-centered care to children who need it most.  Hours:  The hours for this part-time position is 20 hours per week and the preferred schedule is 2.5 days per week.  

• The Social Worker is responsible for providing assessments, planning, interventions and follow-up for patients and family members at Shriners Children's across the continuum of care.
• Through this function, by joining the team, you'll have the opportunity to make an impact on our patient's experience.
• You'll help to provide continuity of care for positive outcomes in our family centered environment.

Required:

• Masters in Social Work
• Ohio Social Work license
• Case Management Certification (CMC, ACM or ANCC) required or the ability to attain certification within 2 years of hire or within 2 years of eligibility
• Three years of social work experience
• Experience conducting and documenting psychosocial assessments and interventions to meet physical, functional, social, emotional, educational needs of the patient/caregivers
• Experience in a healthcare setting with community and national resource navigation and coordination, payment methodologies, mitigating psychosocial barriers to patient/family adherence

Preferred:

• 5-7 years of social work experience 
• Previous social worker experience in a pediatric setting
• Bilingual in English/Spanish
