The Manager of Outpatient and Care Management is responsible for creating safe, healthy environments that support the work of the team to improve patient health outcomes, engagement, and a positive patient experience while minimizing cost to the families and Shriners Children's. The role is influential in creating a professional environment as well as fostering a culture where interdisciplinary team members are able to apply the Shriners FOCUSEDTM Model of care and Care Management guiding principles. Provides service and supervision in the Outpatient Department and Care Management ensuring processes optimize (a) population health, (b) management of care transitions and (c) deployment and adherence to plan of care. Demonstrates knowledge of and expertise in the philosophy and standards of professional care and assists with the application of these to diverse populations with special consideration of age and cultural needs, as well as Shriners Children's requirements. |

Certification/Licensure
RN License in state of practice
Case Management Certification (CCM, ACM, or ANCC) required. If not currently certified, certification by CCM, ANCC, or ACM will be required within one year of hire, or upon eligibility.
Experience
2+ years experience in a Healthcare (medical / surgical) setting
2+ years Care Management
2+ years Pediatrics experience preferred
3+ years management experience in a supervisory/leadership role preferred
