Job Overview

Shriners Children’s, the premier pediatric burn, orthopaedic, spinal cord injury, cleft lip and palate, and pediatric subspecialties medical center, has an opportunity for a RN Care Manager to join our team.

 

The RN Care Manager position requires a thorough understanding of clinical, financial, administrative, and psychosocial components of the healthcare continuum. Must have strong Utilization Management experience. The RN Care Manager is responsible for providing assessments, planning, interventions and follow-up for patients and family members at Shriners Hospitals for Children (SHC) across the continuum of care. These services are provided face to face or telephonically to facilitate the creation of and adherence to a treatment plan to optimize patient health. In conjunction with the interdisciplinary team, this position proactively addresses potential or actual clinical and/or psychosocial needs identified through screening to ensure developmental level of care and health status transitions are safe, seamless and coordinated.

 

This position is Full Time, Exempt and benefits eligible. The pay range for this position is $99,900 - $124,800 annually. Compensation is determined based on years of experience and departmental equity.

Responsibilities

The RN Care Manager is responsible for providing assessments, planning, interventions and follow-up for patients and family members at Shriners Children's across the continuum of care. Through this function, by joining the team, you'll have the opportunity to make an impact on our patient's experience. You'll help to provide continuity of care for positive outcomes in our family centered environment.

An RN Care Manager demonstrates strong assessment skills reflective of ability to identify social and medical inequities in a medically complex patient population.
An RN Care Manager should exehibit working knowledge of compliance regulations, stewardship of patient and organizational resources, transitional care, utilization management and Interqual Criteria. Possess s
trong communications skills-both oral and written is required. Must be able to express Empathy, caring and compassion for children with complex medical conditions and their support systems.  Bilingual in English and another additional language is preferred. 

Qualifications

Education

  • Bachelor’s Degree in Nursing (BSN) is required.
  • Master’s Degree in Nursing (MSN) or related field is preferred

Certifications and Licensures

  • Current RN license in the State of Hawaii required
  • Case Management Certification required;  (CCM, ACM or ANCC) required within 12 months of hire or within 12 months of eligibility
  • Current AHA Basic Life Support (BLS) Certification required

 

Experience

• 3 years of Nursing required
• Minimum of 1 year as an RN Care Manager, pediatric care management experience preferred
