Job Overview

The Physical Therapist provides evaluation and therapy services, including direct patient care, patient/family education and transition of care, in an efficient, age-appropriate, and culturally sensitive manner upon licensed independent practitioner referral.  In this position, you will have the unique opportunity to help our patient through their journey from the moment the child is brought through the door through the time the child leaves to go home.  You'll get to help our kids grow into the new person that they want to be.  You will provide evaluations and therapy services, education and training to meet the needs of our patients.  The successful candidate will get the opportunity to learn and grow in their practice by being exposed to multiple settings.

Responsibilities

  • Upon receiving a licensed independent practitioner referral, implements a systematic patient examination and evaluation utilizing comprehensive compliant and safe clinical care demonstrating the philosophy of family centered care
  • Works with medical staff and the multidisciplinary team to design and implement individualized plans of care to meet the needs of the patient and their family.
  • Makes appropriate discharge plans by collaborating with patients, families and the interdisciplinary team.
  • Functions as a team member and develops leadership skills through activities such as mentoring and providing education to students and other professionals.
  • Supports risk management and compliance initiatives through accurate, thorough documentation.

Qualifications

Education

Bachelors degree in Physical Therapy. 

Certification/Licensure

licensed in state of practice

Skills

1 year physical therapy experience preferred

New graduates considered

Pediatric experience preferred
