Job Overview

Shriners Portland is looking for a Full Time Social Worker to join our team!  The Social Worker position requires a thorough understanding of the clinical, financial, administrative, and psychosocial components of care management.  The Social Worker is responsible for providing psychosocial assessments, planning, interventions and follow-up for patients and family members at Shriners Children's across the continuum of care.  These services are provided face to face or telephonically; to facilitate the creation and adherence to a treatment plan and optimize Patient health in conjunction with the interdisciplinary team; this position pro-actively addresses potential or actual needs identified through screening to ensure developmental, level of care and health status transitions are safe, seamless, and coordinated.

 

Schedule: Full-time  (in-person)

Holidays: 9 observed holidays per year

Compensation range: The hired applicant's placement within the range is based on qualifications, professional experience and education.

Responsibilities

  • Documents assessments, interventions, plans and evaluations in accordance with regulatory agency requirements and SHC policy.
  • Assures prudent utilization of all resources (fiscal, human, environmental, equipment and services) by evaluation of the resources available to the patient and balancing cost and quality to ensure the optimal clinical and financial outcomes.
  • Coordinates the plan of care plan to achieve desired outcomes and integrates the work of the health care team by coordinating resources and services necessary to accomplish agreed upon goals.
  • Advocates by supporting or recommending services for the protection of the patient's health, safety and rights.
  • Arranges services among community agencies, physicians, patients, family, caregivers, and others involved in the plan of care.

Qualifications

Education

  • Bachelor in related field, Required
  • Master's Social Work, Preferred

Experience

  • 3 years Social Work, required
  • 5 years Related work experience, preferred

Certification and Licensures

  • Commission for Case Manager Certification (CCMC) required
  • Basic Life Support (BLS) required
