The Social Worker position requires understanding of common ethical and legal issues in social work practice in health care settings including: bio-psychosocial-spiritual assessment; use of the strengths perspective; client and family engagement in all aspects of social work intervention; case management/care management/care coordination/health care navigation; discharge and transition planning; client concordance with and adherence to the plan of care; Advance Care Planning, Palliative Care, including pain and symptom management, identification of child abuse, trauma, neglect, and exploitation; crisis intervention, facilitation of benefits and resource acquisition to assist clients and families as well as advocacy with other members of the interdisciplinary team and within the health care institution to promote clients and families' decision making and quality of life; client, family, interdisciplinary, and community education; family systems issues, including the impact of health care concerns illness, and disease on family relationships; life cycles, and care-giving roles and support needs. (Pay range for this position is $29.79/hr - $44.69/hr. The hired applicant's placement within the range is based on qualifications, professional experience and education)

• Accountability for the achievement of optimal outcomes within the standard of practice
• Resource Management, assures prudent utilization of all resources (fiscal, human, environmental, equipment and services) by evaluation of the resources available to the patient and balancing cost and quality to ensure the optimal clinical and financial outcomes
• Coordinates the plan of care plan to achieve desired outcomes and integrates the work of the health care team by coordinating resources and services necessary to accomplish agreed-upon goals.
• Advocates by supporting or recommending services for the protection of the patient's health, safety and rights.
• Collaborates by working with patients, families, caregivers, and the health care team.
• Arranges services among community agencies, physicians, patients, family, caregivers, and others involved in the plan of care.

Education

• Bachelor in related field, Required
• Master's in Social Work, Required

Experience

• 3 years Social Work, preferred

Certification

• Social Work license in state of practice (Washington) required
• Current Case Management Certification (CMC, ACM or ANCC) is required (or required within 12 months of hire)
