Job Overview

Shriners Children's Boston has a part time RN Care Manager position available! 

 

The RN Care Manager is responsible for providing assessments, planning, interventions and follow-up for patients and family members at Shriners Children's across the continuum of care. Through this function, by joining the team, you'll have the opportunity to make an impact on our patient's experience. You'll help to provide continuity of care for positive outcomes in our family centered environment.

 

The pay range for this position is $47.54 hourly to $81.50 hourly. Compensation is determined based on years of experience and department equity.

Responsibilities

  • Coordinates the plan of care to achieve desired outcomes and integrates the work of the health care team.
  • Ensures the development of a safe and effective plan of care through early identification and assessment of the patient's needs and the resources available.
  • Collaborates by working with patients, families, caregivers, and the health care team locally and within the SHC system to jointly communicate, problem solve, and share accountability for optimal outcomes.
  • Arranges services among community agencies, physicians, patients, family, caregivers, and others involved in the plan of care.
  • Ensures prudent utilization of all resources by evaluation of the resources available to the patient and balancing cost and quality to ensure optimal clinical and financial outcomes.
  • Other duties as assigned.

Qualifications

Education

BSN required, MSN preferred.

Certification and Licensure

RN license in state of practice

Case Management Certification (CMC, ACM or ANCC), or willingness to achieve within 12 months of eligibility.

 

Required experience:

3 years Nursing experience required

5+ years of clinical experience in Pediatrics, Burn care and/or Orthopaedics preferred.

1+ years Care Coordination/Case Management experience is preferred.

Experience with utilization review/management with a working knowledge of InterQual criteria is strongly preferred.
