Job Overview

Shriners Northern California is looking for a Full Time California Licensed Social Worker to join our team!  The Social Worker position requires a thorough understanding of the clinical, financial, administrative, and psychosocial components of care management.  The Social Worker is responsible for providing psychosocial assessments, planning, interventions and follow-up for patients and family members at Shriners Children's across the continuum of care.  These services are provided face to face or telephonically; to facilitate the creation and adherence to a treatment plan and optimize Patient health in conjunction with the interdisciplinary team; this position pro-actively addresses potential or actual needs identified through screening to ensure developmental, level of care and health status transitions are safe, seamless, and coordinated.

 

Schedule: Full time 1.0 FTE

 

The LCSW pay range is $50.59 - $71.42/hour. Pay is determined based on experience and department equity. 

Responsibilities

  • Bio-psychosocial assessment
  • Crisis intervention
  • Brief Suicide Screening Assessment
  • Participation in Multidisciplinary Rounds/Conferences
  • Patient and family support and education
  • Referrals to community based services
  • Discharge and transition planning

Qualifications

Required

  • CA LCSW
  • Masters Degree in Social Work
  • CMC, ACM or ANCC (within 12 months of hire or eligibility)

Preferred

  • Bilingual 
  • 3+ years pediatric healthcare experience
